1. Introduction
1.1. The Patient Safety Incident Response Plan (PSIRP) sets out how Croydon Health Services intends to respond to patient safety incidents over a period of 24 months (June 2023-May 2025).
1.2. Patient Safety Incidents are any unintended or unexpected incidents which could have, or did, lead to harm for one or more patients receiving healthcare.
1.3. All incident responses are conducted for the purposes of learning and improvement and as such, there is no remit to apportion blame or determine liability, preventability or cause of death.
1.4. This Incident Response Plan is not a permanent plan. CHS will take a dynamic approach in responding to incidents. The trust will also consider emerging patient safety risks, specific circumstances in which incidents occurred, and the needs of those affected (both Staff & Patients).
