What is Patient Initiated Follow-Up?

Some patients with long-term conditions or following treatment don’t always need regular check-ups. Instead, they can be offered something called Patient Initiated Follow-Up (PIFU).

PIFU lets you decide when you need an appointment, giving you control and access to help when you need it.

Rather than having regular clinic visits or check-ups, PIFU allows you to book your own appointment if something changes with your condition, like a flare-up. This can save you time, worry, and trips to the hospital, which we know can interrupt your daily life.

How does it work?
After seeing your consultant, they may decide PIFU is right for you. If so, they’ll put you on the PIFU pathway for a set period of time, which might vary depending on the department. Your consultant will explain how it works, tell you which symptoms to look out for, and give you all the information you need. It’s your choice whether or not to use PIFU.

How do I book a PIFU appointment?
It’s quick and easy to book. If you need a follow-up appointment:

  • Use My Care, the hospital’s online patient system, to send a request. This will be sorted within 7 days.
  • Or, call the hospital department and tell them you’re on a PIFU pathway. The team will help arrange an appointment, usually within the next four weeks.

It’s important to attend your appointment. If you can’t make it, please let us know so we can offer it to someone else who needs it.

When should I call for a PIFU appointment?
Your consultant will tell you what symptoms to watch for and when to get in touch.

When NOT to use PIFU
If you need urgent medical help, contact your GP or NHS 111. If it’s an emergency, go to your local A&E. For other health concerns or if you feel unwell, your GP is still the best person to contact.
