Preparing for your treatment:

1. I have my appointment letter

Arriving at Purley Elective Centre:

1. I have arranged my own private transport or booked the patient transport service to take me to hospital
2. I understand that I will have to wear a face covering when I come into the hospital
3. I know how to get to the Purley Elective Centre
4. I understand that I will not be able to bring family or friends with me to hospital (unless a carer)   

After your treatment:

1. I have read my discharge letter and understand what I need to do to recover at home
2. I know who to contact if I have any questions or concerns about my treatment
