About The Community Children’s Nursing Service

The Children’s Hospital at Home Team (CHAHT) was formed in 1995 and was made up of just 3 Children’s Community nurses. TheCHAH worker team now includes one Matron, twelve Community Children’s Nurses and one Practice Educator. 

The Children’s Community Nursing Team provide a range of services to children and young people who require support in their homes or other community settings. The service is provided to all children and young people with a nursing need, who are registered with a Croydon GP and are aged 0-18 years (or up to 19 years for those with special needs)


We are a team of children's nurses who provide care to:

  • Children with acute and short term conditions
  • Children with long term conditions
  • Children with disabilities and complex conditions
  • Children with life limiting and life threatening illness, including those requiring palliative and end of life care

Feedback

We are constantly striving to improve the service we provide and we value all your feedback. Team members will often bring an iPad to their visits and request a family and friends survey is completed. A questionnaire will also be given to families on discharge for you to complete and return to us.

You can fill in our online patient survey here. If you have questions, concerns or suggestions, it is always best to speak to the nurse who is looking after your child as soon as possible. How to refer

Please ensure your patient has a Croydon GP prior to referral.

How to Refer:

Referrals can be sent to the team via e-mail. Referrals can be made by any health professional 

Please complete all relevant sections of our referral form (found on this webpage) and e-mail it to mhn-tr.chah@nhs.net. 

Please call the team to confirm receipt of referral and capacity to accept

Patients should be discharged from hospital with at least 14 days supply of equipment.
