About us

District nursing teams provide high quality nursing care at home for adults who are sick or have a disability or long-term health condition. They work closely with GPs and social services to plan care tailored to meet the individual patient’s health needs.

Access to the district nursing service is through the family doctor (GP), hospital staff or other health or social care professionals.

District nurses provide the following services:-

  • Management of long term conditions
  • Post operative, traumatic and chronic wound management
  • End of life care 
  • Continuing care assessment
  • Specialist nursing equipment for patients with nursing needs
  • Diagnostic tests and screening
  • Intravenous medication therapy
  • Patient education and health promotion- teaching patients and carers to gain skills to promote self care and to aid recovery from illness. 
  • Medicines management, for example teaching self administration of Insulin
  • Nutritional monitoring and management 
  • Continence management

The service is for patients aged 18 years and over that meet the following criteria:

  • The patient MUST be registered with a Croydon GP (permanent or temporary)
  • The patient’s care needs MUST be health related and require nursing intervention, for example post operative care or wound management.
  • The patient is housebound and has nursing needs.

Contact your GP or Hospital who referred you to the District Nursing Service.

What do I do if I want to change or cancel an appointment?

Contact the District Nursing office that contacted you to make the first appointment.

What should I make available for my appointment?

Any supplies / dressings / letters provided to you by a Hospital or GP

What time should I expect the nurse to arrive for my appointment?

The District Nursing Team will contact the patient with a visit slot and will aim to be with the patient within one hour either side of the slot.

How to refer

District nurses care for patients in their own homes.  There are a number of district nursing centres across Croydon but the Transforming Adult Community Service means that referrals for adult community services can go through a single point of referral

  • Teaching patients and carers skills for independent care

  • Nutritional monitoring
  • Continence management

Referral criteria

Inclusion:

  • 18 years or over
  • Patient must be registered with a Croydon GP (permanent / temporary)

And the patient must meet either one or both of the following criteria:

  • Patient’s care needs must be health related and require short term nursing intervention e.g. post operative care/wound management
  • Patient is housebound and has nursing needs 

Exclusion:

  •  Under 18 years
  • Where nursing care can not be given safely within the patients’ home environment or where the safety of the nurse or patient is compromised

Referrals should be made through TACS (Transforming Adult Community Services).
