About us

The Community Intermediate Care Service looks after people when they have been discharged from hospital or to prevent unnecessary admission, as well as seeing patients at home to reduce their level of dependence on either formal or informal care.

Nurses, therapists and generic support workers offer rehabilitation in patients’ homes from a few days to a maximum of 6 weeks. There are also a small number of intermediate care beds in Croydon in residential care homes and in a nursing home.

The service is appropriate for any adult with a Croydon GP, or who lives within the borough, who needs this kind of rehabilitation support and who is willing and able to participate in this type of programme.

Within CICS there are specialist staff from the Community Stroke Service and the Integrated Falls Service, and our ‘liaison’ colleagues support the community nurses and Accident and Emergency team at Croydon University Hospital.

An elderly care team at Croydon University Hospital provide support to help us give the best possible care to patients outside of hospital.
